ULS Fine Appeal Request Form

Library borrowers may appeal fines on loaned items if they believe the fines were created in error, or if the borrower experienced extenuating circumstances that warrant an exception. Please note that appeals must be made within a 60-day window from the fine’s date of issue. Also note, the following are not considered valid reasons for appeal:

  • Lack of knowledge of Library policy
  • Disagreement with the Library fine or fee structure
  • Inability to pay fees and charges
  • Material loaned to a third party
  • Non-receipt of Library courtesy emails
  • Forgetting the due date
  • Semester breaks, leaves, vacations, exams

The library will decide on appeals within two weeks of receiving the request form and notification will be sent by email.
